Getting Reports Ready to Apply: Payroll Protection Program (PPP) Loan

The operating data required when you submit an application for a PPP loan is entirely based on your payroll costs. Those costs will be provided on the following reports, all of which should be available via your payroll provider’s self-service portal.

• IRS Form 941 for all available quarters from 2019 and 2020

• IRS Form 940 or IRS Form 944 for 2019

• Payroll Detail Reports by month: obtain both 2019 and 2020

- Confirm these monthly reports include these categories of expense:

- Gross Wages (detail)

- Health care contributions (if applicable)

- Employer Retirement Contributions (if applicable)

- State and Local Taxes on compensation (i.e., unemployment taxes)
